Depends on your pain threshold. but I'm guessing like any piercing, it'll hurt for a while.

I don't have my bottom lip pierced but my sister has and she said it did hurt. But I dunno, I think it depends on the person.
I have my top lip pierced, and I am guessing it might be similiar to the bottom lip, and it didn't hurt at all.
